Application packets need to be completed and submitted along with a $125 per family registration fee. 

The following documentation will also be needed:

Child’s birth certificate

Child’s baptismal certificate (if applicable)

Your child’s most recent report cards, standardized test scores and IEP’s developed (if applicable).  If transferring from another school, complete  the Request for Records form.

Your child’s most recent health forms

All families must register for FACTS Tuition Management.

FACTS is used to collect tuition as well as apply for financial assistance.
